Online Feedback Forms

The Dietitians Board offers both the public and registered dietitians the ability to provide feedback online.

We offer two types of online forms to provide feedback:

General Feedback
This is intended for general feedback - anything from compliments, to suggestions, to complaints about the Board and the services we provide.

Report a Problem
This form is designed for you to be able to provide us with details of any issues you are experiencing with our online systems. This might be our public website that you are on now, and for registered dietitians it may be an issue you are experiencing in your secure Practitioner Portal. The goal is that your information will help us and our IT providers to identify and fix system issues, to improve the overall experience for yourself and other users going forward. If you are experiencing an issue online that needs urgent attention, such as inability to renew your practicing certificate online, contact the Board's office directly.

 

Please note that these are feedback forms, and are not contact forms. We review submissions via these forms periodically or as required, and do not promise an immediate response to your submission, although we aim to rectify any online issues as quickly as we can.

This form is securely encrypted by Google Forms, and your responses can only be viewed by Dietitian's Board staff (or our Software Support team if required). Even so, as a best practice measure, please do not provide sensitive information such as credit card details or passwords in these forms.
